Post-acute care supports recovery or management of chronic conditions, often involving rehabilitation or palliative services after or instead of hospital stays. Depending on care needs, treatment may include nursing homes (skilled nursing facilities). Florida's nursing homes now serve a more diverse patient base, offering a wider range of acute care, rehabilitation, and convalescent services.
